We invite you to submit a complication case as mentioned previously.

Case Subission Guidelines:

• Cases must be written in English.

• Submitting more than one case is acceptable.

• Cases must be SUBMITTED BY 11:59 p.m. (Malaysia time), on FRIDAY, 20th May 2016. Late submissions will not be accepted.

Format:

• 1. Title: Type the title in CAPITAL LETTERS.

• 2. Authors and Affiliations: The first author listed should be the presenter in person. Type the authors’ names in full. When more than one author is listed, use superscript numbers to match authors and institutions. Capitalise words only as required. The list of author names should be followed by the author’s institutions, city or state and country.

• 3. Case summary should have the following contents:

• Introduction of the case

• Objective

• Description of the problem, procedure, technique and equipment's used.

• Results
